She's smoked for pretty much 13 years, it's getting bad now

No but she has bad smokers cough..
My step dad has fits where he can't stop coughing.
Smoking is just disgusting to me

Same to me, but it's so hard to let loose of an addiction, it's a really sad thing to witness...someone dying before your very eyes, but somehow they can't get enough of the death they're experiencing.
