Look at the sample application please. Asking politely isn't going to work.
You have to explain what happened, why you were banned, and make sure you know what you did was wrong.

I think he let his friend use his profile because his friend didn't have one, and his friend got him banned. Now his friend made his own minecraft profile and the original bacon wants to be un banned
